18.5 hours per week
until 31 March 2023 + 12 months subject to performance
Salary: £24,960 pro rata
Plus 6.6% matched pension contribution
Funded by Bradford Districts CCG from the Reducing Health Inequalities in City, CLICS (Central Locality Integrated Care Service) is about helping members of the community access support to better self-manage their health and wellbeing and long-term health conditions. The successful post holder will be a member of the CLICS team, a new service, operating across the three Community Partnerships and will work in and take referrals from designated GP practices/Primary Care Networks and key Community-based stakeholders.
CLICS Community Development Worker will:
- Lead on mapping and building of community health assets
- Work with groups to establish and develop community resources
- Support groups to operate health creation activities
- Support groups to establish and grow
Applicants should have:
- Strong links to and knowledge of the voluntary sector
- Knowledge of a wide range of health issues
- Experience of working in a multi-cultural environment with disadvantaged sections of the community
- Knowledge of an Asset Based Community Development approach
